How old were you when you attended this facility?
Lela: 23

Describe your addiction:
Lela: I was on crack cocaine and marijuana

Why did you choose to go to rehab?
Lela: So I can get my life together. I almost lost my family and friends also my life being addicted to drugs

Why did you choose this facility?
Lela: My probation offer choose this program for me because he said this's the best faculty out here in Rockford IL

How long were you in this rehab facility? Inpatient or outpatient?
Lela: for a year and it was outpatient

What is this facility's approach to the treatment process?
Lela: I had to go to treatment it was recommend by my potation officer. I had to do a drug screening and I had to do a one on one section and a group section 3 times a week for a year

What do you think this rehab facility does well?
Lela: The staffs care about you and want you succeed and life. The staffs will be right by your side until the end. All Im trying to say they put there feeling into there jobs and I appreciate that

What could this facility do better?
Lela: IF they would lower they prices down it will be great for the people who cant afford those prices

Overall do you feel your treatment here was successful or not?
Lela: Yes it was very successful They kept my head on my shoulder and I am 5 yrs clean now have my family and friends a degree a husband a job I mean it really change my life

Did you relapse after treatment?
Lela: No still go to meeting time to time

Would you recommend this facility to a friend or a family member?
Lela: yes I will recommend this facility to my family and friends The staff will put your life in order

What general advice would you give to somebody dealing with addiction?
Lela: I will say it is very very hard dealing with addition you just have to take it one day at a time love yourself go to meeting s and find a higher power.

Lela: I have been in no other rehab facilities.
